Prior to your appointment
Please complete your registration and health history forms from home prior to your first appointment, or arrive at least 15 minutes early to complete them in office.
At your first appointment
Your first appointment will be a consultation, where we will review your dental and medical history, your treatment and sedation options, and address any questions or concerns you may have. If we need additional x-rays, we will take them at this time. We will then schedule the appointment for your oral surgery procedure.
- All patients under the age of 18 will need to be accompanied by a parent or legal guardian.
- A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours prior to your surgery. You will also need an adult to drive you home.
- Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

Insurance Policy
We are pleased to announce that we are now a participating provider and “In Network” with Northshore HMO Site 447 as well as the following DENTAL PPO Insurance plans.

We will submit the claim to your insurance company on your behalf and provide you with an estimated out of pocket cost prior to treatment. Dental insurance plans are designed to contribute to the overall cost of your treatment, but insurance companies usually do not pay the full amount of your treatment. If your insurance company pays less than the estimated amount, you will be responsible for the remaining balance.

We require at least 24 hours notice to avoid cancellation fees.
What if I my dentist just took x-rays?
Before proceeding with treatment, we take a CBCT (Cone Beam Computed Tomography) scan to obtain a highly detailed 3D image of the patient’s surgical area. This advanced imaging technology provides a comprehensive view of the bone structure, teeth, nerves, and blood supply, allowing us to plan the procedure with precision and ensure the highest level of safety and effectiveness during surgery. It also helps us assess the surrounding areas of the head and neck for a complete and thorough evaluation.
Do you accept payment through flexible spending accounts (FSAs) or health savings accounts (HSAs)?
Yes, we accept FSA and HSA cards for payments. Please check with your benefits provider to ensure the expenses are eligible under your plan.
What if my insurance doesn’t cover the entire cost of my procedure?
If your insurance doesn’t cover the full cost, we’ll work with you to review payment options. We offer flexible payment plans and financing options to help you make your treatment affordable. We have two third party dental patient financing options:
Will you take me as a patient even though I do not have dental insurance?
Yes. We welcome all new patients. We have a variety of affordable payment options available to our patients. We gladly accept cash, Visa, MasterCard, American Express and Discover credit cards.
CareCredit and Proceed Dental Patient Financing is also available for patients who need extended payment options.
What happens if I’m running late to my appointment?
If you’re running late, please call our office as soon as possible. We’ll do our best to accommodate you, but depending on our schedule, we may need to reschedule your appointment to avoid delays for other patients.
Can someone else bring me to my appointment if I can’t make it myself?
Yes, another adult can accompany you, but for patients under 18, a parent or legal guardian must be present. For surgical procedures, we recommend having a responsible adult drive you home and stay with you as you recover.
Can I reach the ORal Surgeon if I have a dental emergency after hours?
Yes. If you are already a patient of record and you have a dental emergency after hours, please call us at 847.998.8959. Your message will be forwarded to the doctor who will contact you as quickly as possible.
